| 8. | Recommended precautions to include evacuation distance if appropriate. | ||
| 9. | Actions taken (police, fire, EMS, and EMA-Community Emergency Coordinator called). | ||

To report an incident to the State of Indiana, call the Indiana Department of Environmental Management, Environmental Protection Division. For immediate technical advice, call the Chemical Transportation Emergency Center ( CHEMTREC ). **The State and LEPC must be notified.** Emergency notification requirements involving transportation incidents will be satisfied by calling 911 or, in the absence of a 911 emergency number, calling the operator. |
